SOLTERRA TEAM SERVICES LLC is Hiring a Move in Coordinator Near Phoenix, AZ

Move in Coordinator is responsible for the coordination of admissions for the community. Coordinates between community referral sources and community availability to ensure seamless transfers based upon needs and opportunities for placement. Gathers information which includes a physical assessment of potential admissions. Pre-qualifies for Medicare, Medicaid and third party insurance coverage.

ESSENTIAL JOB DUTIES:

  • Handles the patient admissions function, bed assignments, and completion of preliminary paperwork for entering patients. Gathers documentation pertinent to resident health care and needs in preparation for admission.
  • Works in tandem with medical, nursing, clinical liaison and business office staff to ensure appropriate patient placement and coordination of their services at their arrival at the community.
  • Coordinates resident Medicare, Medicaid, Managed Care and Private insurance benefits including beneficiary consent, denials, explanations and utilization.
  • Collects, reviews, and inputs clinical patients data into a computer processing system.
  • Completes admissions contract with patient or responsible party and explains community policies and procedures were appropriate.
  • Handles inquiries, conduct tours and explains the admissions process.
  • Monitor daily discharges, admission to update management.
  • Follow up on all referrals daily to check current status.
  • Complete referral tracking worksheets.
  • Assists with discharge planning for homeward bound residents. Contacting and utilizing community resources on the resident's behalf and serving as a link between the resident and these resource systems when necessary.
  • Other duties as assigned.


EDUCATION/EXPERIENCE:

  • Experience preferably in a long-term care facility
  • Knowledge of Medicare admissions process
  • Able to lift a minimum of 25 to 75 pounds.

SKILLS:

  • Good communication skills, ability to write, speak, read and understand English.
  • Team player.
  • Highly organized.
  • Ability to taste and smell food to determine quality and palatability.
  • Ability to handle and manipulate equipment in supervising, demonstrating and assisting in food preparation and serving.
  • Knowledge / experience of health and safety regulations.
  • Possess a positive attitude with working with the elderly and special needs residents.

EXPERIENCE/SKILLS: $17.00

  • Minimum one (1) year marketing and sales experience in a healthcare setting or homes/apartment sales and rentals with significant level of responsibility and accountability for goal achievement.
  • Ability to conduct strategic market and demographic analysis within the community Primary Target market
  • Ability to develop strategic marketing plan for community
  • Demonstrated ability to set and achieve census and financial targets.
  • Strong communication, planning and organizational skills
  • Flexible schedule, ability to work nights and weekends
  • Able to lift a minimum of 25 to 75 pounds.
  • Working knowledge of Microsoft Office Word, PowerPoint, Excel and Outlook.
