Position: Housing Case Manager (Hybrid)
Locations: Amityville, NY
Department: Homeless/Veteran Services
Hourly Rate: $22/HR

Schedules: 8am - 4pm or 9am-5pm - Monday - Friday (2 in office days per week)

Overview: The Housing case manager carries a caseload of 25 participants, and creates individualized plans designed to help participants meet specific Permanent Housing goals.

Requirements:

• Bachelor's degree
• Two years of experience in Case Management/Housing
• Must be comfortable with Traveling, 40% field Work (Travel Compensated)

Responsibilities:

• Establish linkages and maintain solid working relationships with supportive housing providers, real estate brokers, managing agents, and landlords as a resource for consumers.
• Provide supportive social services including individual and group counseling, workshops, education, referrals, and escorting in an effort to prepare consumers for successfully securing and maintaining permanent housing.
• Prepare and assist consumers with the housing application/interview process.
• Provide accurate, complete, and timely documentation of the provision of supportive social services and consumer progression/regression toward the goals of securing permanent affordable housing.
• Attend housing-related DHS meetings.

Pay: $22.00 per hour
