Overview:

We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will have experience in critical care, diagnostic evaluation, and discharge planning. As a Registered Nurse Case Manager, you will play a crucial role in providing patient care, particularly focusing on geriatrics and behavioral health.

Responsibilities:

- Conduct comprehensive patient assessments and develop individualized care plans
- Coordinate with healthcare providers to ensure quality patient service
- Manage and oversee patient care to promote optimal outcomes
- Utilize case management techniques to facilitate efficient healthcare delivery

Job Type: Full-time

Pay: $61,250.00 - $75,000.00 per year
