Solaris Healthcare Forest Lake is now hiring a Transition Care Coordinator . As the Transition Care Coordinator , you are responsible for placement of clients and referral generation, assessment, and conversion of clients in hospital or community setting.

We are located at 3355 E Semoran Blvd, Apopka, FL 32703.

PRINCIPAL DUTIES AND JOB RESPONSIBILITIES :

• Increase referral rate from area hospitals.
• Develops relationships specific to area to assist in maximizing census.
• Maintains knowledge of current regulations governing Medicare, Medicaid, Insurance.
• Maintain effective communication with company personnel.
• Embrace the company’s core values and incorporate them into the daily job.
• Demonstrate respect and compassion in every interaction.
• Perform other Duties as assigned.
• Onsite reviews
• Responsible for promoting the Facility image in the community. Dress professionally and conducts business in an efficient and professional manner.

KNOWLEDGE AND SKILLS REQUIRED :

• Ability to work independently.
• Exceptional people skills
• Excellent verbal, written, and interpersonal communication skills.
• Ability to deal tactfully with personnel, residents, family members, and the public.
• Time management
• Ability to prepare accurate reports.
• 2 years of relevant experience in Health Care Marketing or Business Development
• Bachelor’s degree
• Must be 18 years of age or older.
