Job Overview:

We are seeking a dedicated Registered Nurse to join our healthcare team. The ideal candidate will have a passion for patient care and possess a diverse skill set in various medical settings. This nurse would be covering on-call every 3rd weekend and float 2-3 days during the week. The On-call weekend would be Friday-Sunday (5pm-9am).

Responsibilities:

- Provide direct patient care in homes and nursing homes

- Conduct triage assessments to prioritize patient needs

- Provide End of Life Education to patients and families/caregivers

- Deliver acute care services with compassion and professionalism

- Proficient in catheterization procedures

- Experience working in nursing home facilities is a plus

- Ability to handle patients with behavioral health needs

- Ability to work independently

Experience:

- Valid RN license in the state of practice

- Minimum of 2 years of experience in a clinical setting, Hospice experience is a plus.

- Certification in hospice palliative care is a plus

- Strong communication and interpersonal skills

- Ability to work collaboratively with a multidisciplinary team
