We are seeking a compassionate and experienced Registered Nurse to join our Home Health team. As a Home Health RN, you will have the opportunity to provide high-quality care to patients in the comfort of their own homes. Responsibilities:

- Provide direct patient care to children with disabilities.

- Provide tracheostomy care as needed.

- Administer medications, including injections, as prescribed by physicians

- Monitor patients' vital signs and report any changes or concerns to the healthcare team

- Provide education and support to patients and their families on managing their conditions at home

- Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care

- Utilize case management skills to ensure continuity of care for patients

- Document all patient interactions, interventions, and outcomes accurately and timely

Experience:

- Minimum of 2 years of experience as a Registered Nurse in a hospital or home health setting

- Experience working with children is preferred

- Solid understanding of anatomy and physiology principles

Qualifications:

- Valid Registered Nurse (RN) license in the state of [LOCATION]

- BLS certification required; ACLS certification preferred

- Excellent communication and interpersonal skills

- Ability to work independently and prioritize tasks effectively
