Qualifications: 2 years experience inlong term care preferred, Licensed social worker required, must possessexcellent communication and organizational skills

As a Licensed Social Worker in a GraceManagement Services facility you must be able to perform the followingessential functions as demonstrated by the ability to:

Essential Job Functions:

Develop one-to-onerelationships with residents and families.

Interact with residents andfamilies dealing with feelings about death or dying and other emotional,mental, environmental or physical limitations.

Establish relationships witharea hospice agencies.

Complete the social serviceportion of the MDS within 14 days of admission.

Document the social carecomponent of the Comprehensive Care Plan for each resident as identified by theinterdisciplinary team within 7 days of completion of the MDS.

Document all interactions withresident and/or family in the assessment and care plan as required by statestandards.

Document the social andemotional needs of residents.

Participate in utilizationreview meetings.

Act as a liaison with social,health and community agencies.

Be responsible for referral ofresidents to social, health and community agencies and ensure documentation inthe resident’s record concerning the result of such referrals.

Coordinate family andcommunity resources as needed to solve financial needs and to promote emotionalsecurity.

Take part in in-serviceeducation and attend all in-service sessions relating to the Social ServiceDirector.

Participate in admission ofresidents as assigned.

Coordinate with ActivityDepartment to meet socialization needs of the residents.

Oversee coordination ofreferrals for ancillary services ( dentist, eye doctor, podiatrist,psychologist, psychiatrist, audiology).

Participate in plan of caremeetings.

Maintain log of misplacedproperty and ensure follow-up.

Participate in Resident’sCouncil meetings at least once a month.

Perform other related dutiesas directed by the Administrator.
