We have a great new opportunity for an LCSW for our growing organization located in Lower Fairfield County Connecticut. The LCSW will have an opportunity for career advancement. You will be working with an organization that has recently become a major provider of home health related services part of a larger healthcare organization. Title: Home Care LCSW

Location: Norwalk, CT

Compensation: $75,000 to $80,000

Responsibilities for Home Care LCSW:

Collaborate with professionals to assess patients physical and mental conditions

Performing routine home visits and checkup appointments

Intervening in crisis situations

Referring patients and family members to different community service programs

Modifying treatment plans and counseling

Requirements for Home Care LCSW:

Must be licensed LCSW

Prior experience in home health preferred

Preferred MSW degree

Required Bachelor’s degree

Clean driving record

Job Type: Full-time

Pay: $75,000.00 - $80,000.00 per year
