Location and Position Details

• Location: White Plains, USA
• Experience: 3 – 20 Years
• Shifts: 13 shifts/month, alternating weekends and holidays, 10:00 AM - 10:00 PM

Position Summary

• Provides social work services to patients and families, including assessing medical psycho-social programs and formulating treatment plans.
• Works in cooperation with other healthcare team members to maintain patient safety.

Essential Functions and Responsibilities

• Conducts interviews to determine continuing care needs.
• Completes social work comprehensive assessments.
• Provides psychosocial information for patient transitions between units.
• Offers supportive counseling and crisis intervention to patients and families.
• Designs and implements treatment/continuing care plans.
• Coordinates interdisciplinary case conferences as needed.
• Assists in presentations, training, and in-services.
• Responds to external and internal disasters when assigned.

Education and Experience Requirements

• Master of Social Work Degree from an accredited institution.
• Licensed Social Worker (LMSW/LCSW) in New York.
• Two years of acute care hospital setting or related experience.
• NYS SCREEN Certification within 3 months of hire date.
