Sixteenth Street Community Health Centers is Hiring a CADT Psychotherapist or Qualified Treatment Trainee Near Milwaukee, WI

  1. Promote healthy personal and family relationships through education, exploration of relationship dynamics, and therapeutic intervention.
  2. Conduct scheduled therapy sessions with clients; additional sessions are provided on “as needed” basis.
  3. Develop and implement therapeutic groups for the patients, including goals, interventions, and desired outcomes.
  4. Participate in weekly multidisciplinary case reviews, psychiatric consultations, and consultation with team members on cases as needed.
  5. Document treatment notes on each active client as dictated by DHS 40.
  6. Engage in weekly clinical supervision by the clinic coordinator or other licensed staff as required by DHS 40.
  7. Apply principles of behavior modification, social skills, cognitive therapy and other evidence-based practices to the child and adolescent day treatment setting.
  8. When appropriate, assist in restraint and seclusion if/when patients become physically aggressive.
  9. Provide each client with a custom-designed treatment plan which will include measurable, realistic and appropriate treatment goals, and outline specific therapeutic interventions utilized, ongoing assessment of progress, modifications when necessary, and aftercare planning.
  10. Continuously review client treatment plans and record therapy sessions with progress notes in an electronic health record. Ensure notes contain accurate and relevant information and stay within legal parameters.
  11. Document treatment reviews on each active client as dictated by DHS 35.
  12. Engage in weekly clinical supervision as required by DHS 35 and DHS 40.
  13. Collaborate with referents and community resources and make requested court appearances as they pertain to the client’s continuity of care.
  14. Comply with SSCHC TB Control Plan including PPD testing.
  15. Perform other duties as assigned.
QUALIFICATIONS:
 
  1. Possess a master’s degree in counseling, social work, or psychology.
  2. Possess a minimum of a practice license (APSW or LPC-IT) consistent with one’s graduate training.
  3. Ability to relate well to people from diverse ethnic and cultural backgrounds.
  4. American Heart Association Health Care Provider CPR certification or American Red Cross Professional rescue is required within thirty (30) days of the date of hire. Annual re-certification is required.
  5. Formal training in management of aggressive patients is required within sixty (60) days of the day of hire (this may be provided during the orientation process). Annual recertification is required.
  6. Familiarity with DHS 40.
  7. Bilingual (Spanish/English) required.
