Description
This position will provide peer recovery support services to patients of SIU Center for Family
Medicines Medication Assisted Recovery (MAR) Clinic. This position will work as an integral
part of the MAR team to serve as a resource and assist patients in meeting recovery goals.
Examples of Duties
Recovery Support Direct Patient Interaction 100%
1. Assists patients in setting and achieving personal goals in person, by phone or in
group settings.
2. Draws upon own personal recovery experience and challenges as a basis for
engagement as a peer.
3. Implements wellness and prevention initiatives.
4. Facilitates health education and / or recovery-oriented groups.
5. Responsible for providing consistent communication to MAR team to evaluate patient
status, ensuring that provided information and reports clearly describe progress.
6. Participate in monthly review of care plans in conjunction with the MAR team to
monitor patient success and compliance with treatment goals.
7. Through skill building intervientions and other supportive interventions, assist patients
in effectively utilizing a wide spectrum of resources, managing symptoms of their
illness, and developing daily living skills to gain greater independence, employment
and other meaningful activities.
8. Connect patients to community resources including but not limited to housing, medical
care, education, vocational training, and additional recovery resources / meetings.
9. Establishes and maintains relationships with other recovery-oriented services and
workgroups and initiatives, as requested.
10. Assist patients with transportation to MAR related appointments.
Qualifications
1.High school diploma or equivalent.2. One (1) year (12 months) of ''lived'' (or extensive ''lived'') experience in sustained recovery and reentry in these areas of specialization or of work experience : A.
Substance abuse (illicit drug or alcohol dependency)
3.Eighteen months (18 months) of work experience comparable to the Peer Support Assistant of this series.
4. Possession of a valid and current (appropriate) State of Illinois driver's license, and proof of insurance at the time of appointment.
Note : The possession of a valid and current Certification in the area s) of specialization inherent to the position from an accredited organization may be substituted for #3 above if not used to meet the Minimum Acceptable Qualification.
