JOB STATUS:

Full-time position: Monday - Thursday (7:30a to 6p)

The Family Navigator serves as an intermediary between the provider, social services, and the community to facilitate access to services in support of families and assist in the obtaining of care and resources needed from initial intake and beyond the diagnostic period to meet the ongoing needs for the family and child. Navigators reduce barriers that keep patients from getting timely treatment by identifying patient needs and directing them to sources of clinical, financial, or social support.

QUALIFICATIONS

• EDUCATION: High school diploma or equivalent. A minimum of an Associate degree (in related filed) may substitute for the experience)
• EXPERIENCE: Minimum 3 years of experience assisting patients and families or other related healthcare/community experience. Knowledge of state social service agencies and community resources. Preferred experience in early childhood settings, pediatric healthcare or a school system.
