JOB DESCRIPTION SUMMARY

The AHSD provides leadership for resident care and the medication program in the assisted living and/or memory care neighborhoods under the direction of the HSD and the Regional Health Services Director Registered Nurse, as applicable by State law, to promote the health and wellness of the resident population. This position is responsible for directing resident's health concerns, including but not limited to resident assessments, medication/pharmacy management, resident activities of daily living, and personal care needs. Abides by and upholds company core values.


ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

The AHSD must exhibit the following competencies:

SPECIFIC DUTIES AND RESPONSIBILITIES

  • Leadership
    • Assist the HSD in leading the community's resident care and medication programs.
    • Assist the HSD and business office manager in completing new hire paperwork, monitoring time sheets, and performing medication associate evaluations per company policy.
    • Assist the HSD and collaborate with the business office manager on the level of care and ancillary charges for residents to ensure they are being charged appropriately.
    • Complete and/or oversee scheduling for caregivers, nurses, and/or medication aides.
    • Assist HSD in appropriately staffing resident care and medication staff according to state regulations and organizational guidelines.
    • Participate in clinical and staffing on-call schedule as required.
    • Participate in the Manager on Duty call schedule.
  • Resident Care Program
    • Coordinate and collaborate with the HSD to ensure the following are completed and assist as needed.
      • Provide training, supervision, and monitoring of caregivers in following the resident service plan and completing tasks for the assigned services.
      • Review the resident's medical report before move-in to ensure that the community can accommodate their needs per company policy and state regulations for their individual needs and that regulatory requirements have been fulfilled.
      • Initiate resident service plans according to the resident's individualized needs as prescribed by the ordering healthcare provider in conjunction with the HSD.
      • Ensure caregivers complete daily services and that service plans are updated swiftly when changes are reported.
      • Manage residents' health care, including disseminating information to families, staff, healthcare providers, and third-party providers.
      • Perform on-site assessments of residents admitted to hospitals or skilled nursing facilities for treatment and maintain contact with the residents' families and discharge planners to coordinate the resident's return to the community.
      • Work collaboratively with the Community Relations Director with resident move-ins and complete resident pre-admission assessments timely.
      • Meet with residents and family members regularly for care plan review and level of care changes and answer health care-related questions.
      • Evaluate resident’s weight and skin and obtain physician orders for treatments and interventions as needed, working with third-party providers.
      • Ensure ancillary medical services such as podiatrist, healthcare provider visits, dental visits, psych visits, ambulance, etc., are scheduled and followed through.
  • Medication Program
    • Understand the medication program and assist the HSD as necessary in the following:
      • Provide training, supervision, and monitoring of medication aides and/or licensed nurses in the assistance and/or administration of medications, including but not limited to auditing medication sheets, medication passes, shift change MAR audit, and narcotic counts.
      • Assure that medications prescribed have orders, are available, and are transcribed correctly in the EMAR by frequently auditing the medication records and chart and ensuring proper documentation for medication assistance administration.
      • Maintain ongoing communication with the resident, family, physician, and pharmacy regarding the resident’s medication needs, etc.
  • Memory Care Program (if applicable)
    • Become proficient in dementia/Alzheimer's resident care and the Meaningful Moments program.
    • Assist the HSD as necessary in the following:
      • Train memory care staff in collaboration with the Life Engagement Director over activities, life stories, and redirection techniques to better serve the residents.
      • Work with the Life Engagement Director to ensure the Memory Care environment is inviting, residents and family members are engaged, and staff is appropriately trained.
  • Miscellaneous
    • Communicate regularly with other disciplines involved in resident care, including on weekends.
    • Must take call nights, weekends, and holidays as scheduled. May have to cover caregiver or medication aide/licensed nurse shifts if needed.
    • Participate in the manager-on-duty schedule.
    • Complete all assigned duties that may change from time to time according to resident needs, staffing levels, and working circumstances.
    • Member of the community's emergency response team.
    • Maintain a professional appearance and good personal hygiene per company policies.
    • Perform other duties as assigned

WORKING CONDITIONS

    • Able to work full-time with assigned weekend work.
    • Available by phone 24 hours daily, seven days weekly, as assigned for clinical/staffing needs.
    • Must be able to work with residents with active cases of COVID-19, as required.
    • Must be able to wear face coverings, including required fit testing for N95 respirator.

This position is paid a salary intended to cover all hours worked during the week, knowing those hours may sometimes exceed eight hours in a day or forty hours in a week and may fluctuate from week to week.


QUALIFICATIONS

  • Minimum one year of experience in senior care or related health programs, preferably in an assisted living or memory care setting.
  • Must be able to read, write, and speak English for safety and work instruction purposes.
  • Must be computer proficient in typing and various programs, including a background in EHR management.
  • Subject to a minimum criminal background check.
  • Must desire to work with seniors and tolerate behaviors associated with dementia and other aging processes.
  • Hold a current unencumbered license in the state the employee resides in as a Licensed Vocational/Practical Nurse or Registered Nurse.
  • Hold current CPR certification.
  • Valid driver's license with reliable transportation for resident assessments and community marketing events.
