• Two (2) years of hospital nursing experience in an acute care setting
• One (1) year of Case Management and/or Utilization Review experience is strongly preferred.

Education:

• Graduate of a RN Program
• Bachelor's Degree is preferred

• Monday - Friday 8:00am-4:30pm

Position Summary:

Case Manager will be responsible for assigning, monitoring, and organizing cases for Social Work.

Essential Duties and Responsibilities:

• Assessing clients' physical and mental wellness, needs, preferences, and abilities and using these to develop tailored care plans.
• Working with clients, family and friend support networks and healthcare professionals to put care plans in place.
• Listening to clients' concerns and providing counseling or intervention as required.
• Recording clients' progress including charting referrals, home visits and other notable interactions
• Evaluating clients' progress periodically and making adjustments as needed to improve outcomes
• Following up with discharged clients to ensure they were satisfied with services and they are still in good physical and mental health.
• Report abnormal changes or patterns in nursing staff.
• Completed documentation of required forms/documents.
• Subject to irregular hours.
• May be required to wear personal protective equipment as directed.
• Follow infection control and isolations guidelines.
• Adhere to all hospital polices and procedures.
• Perform other duties as assigned.

License/Certification:

• Current Mississippi RN License is required.
• BLS is required.

Physical Requirements:

The Case Mangers must have the ability to move around freely, walk and stand for long periods of time; use stomach and lower back muscles to support the body for long periods without getting tired; use hands to handle, control, or feel objects, tools, or controls; bend or twist their body; understand the speech of another person and speak clearly so listeners can understand; and see details of objects that are less than a few feet away and at a distance.
