Case Manager/Social Worker

SilverSource Case Managers report to the Director of Social Work, and perform a range of tasks to achieve the best outcomes for their assigned clients.
Responsibilities
· Consult with clients and their families in the office, home or senior site as appropriate to provide assistance and referrals.
· Administer a comprehensive intake evaluation to assess clients’ physical and mental wellness, critical needs, preferences and abilities and vulnerabilities,
· Develop a plan of action with the client to address challenges and set goals to achieve maximum independence, connecting to federal, state and local programs where appropriate.
· Provide services such as financial assistance, application support, budgeting help, and/or referrals to outside agencies/providers.
· Liaise with outside agencies such as Protective Services for the Elderly, City agents, Dept. of Mental Health and others as needed for collaboration on client care.
· Develop and maintain positive collaborative relationships with community agencies and professionals to enhance resources to meet the needs of clients
· Actively participate in interdisciplinary planning team meetings and staff meetings
· Participate in weekly case management meetings, and case supervision consults
· Utilize the agency’s electronic record system to ensure proper and timely completion of documentation, program statistics, and outcome data
· Reach out to clients periodically to check on their wellbeing, goal progress, and address any new challenges
· Make internal referrals where appropriate, for example transportation and volunteer outreach
· Other related activities as assigned
Requirements
▪ Bachelor’s degree required- MSW Social Work preferred
▪ Experience working with the senior population is preferable
▪ Must be proficient in MS Office and be able to maintain client records in keeping with agency standards
▪ Must be a team player who can adapt to evolving agency needs
▪ Must be detail-oriented and have the ability to multi-task

Work Environment
This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, fax, photocopiers, scanners and associated software applications.

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms. This position requires the ability to lift office products and supplies, up to 20 pounds.
Position Type/Expected Hours of Work
This is a full-time position, and hours of work and days are Monday through Friday, 9:00 AM to 5:30 PM with ½ hour for lunch.
