Silver Lake Regional School District is seeking a Program Coordinator for the Pathways Program. The Pathways Program is a postsecondary transition program that serves students 18-22 years of age who have intellectual disabilities, Autism, Neurological and/or Multiple Disabilities. Responsibilities to include but not limited to community outreach, job site coordination, daily lesson planning focused on transition skills, writing IEPs and chairing annual meetings, completing 688 referrals, meeting and communicating with outside agencies (DDS, MRC), managing team of job coaches, arranging community outings, working with high school staff on identifying future students, direct teaching in daily living skills and travel training, and other responsibilities to be assigned by Special Education Coordinator and/or Director of Student Services. License credentials include Severe Disabilities with Transition Specialist endorsement preferred. 7D certified also preferred.
