It’s the way you want to be treated. Summary: Provides leadership in areas of case management, utilization review, discharge planning, and resource utilization. Duties and Responsibilities: Collaborates with Health Care team to coordinate services and facilitate outcomes for patients and families Participates in development, implementation and variance analysis of care delivery standards and outcome measures, i.e. protocols, clinical pathways, quality improvement indicators and criteria Communicates with insurance companies to secure patients stays and resources upon discharge to include lower level of care, durable medical equipment, ambulance services and follow up appointments Facilitates and initiates appropriate resource utilization by evaluation appropriateness of admission , continued stay first level medical necessity screening and discharge planning by matching patient needs to available resources Knowledge of governmental conditions of participation and ability to ensure compliance. Requirements: MSW required, LCSW required (or in process of obtaining) or BSN, MSN preferred. Case Management or discharge planning experience required in the hospital setting.
