Job Overview: We are seeking a dedicated and compassionate Developmental Disabilities Case Manager to join our team. As a Case Manager, you will be responsible for providing support and services to individuals with developmental disabilities. This role requires strong program management skills, as well as the ability to work with a diverse population and collaborate with various stakeholders. Duties: - Conduct assessments to determine the needs and goals of individuals with developmental disabilities - Develop and implement individualized service plans - Coordinate and monitor services provided to clients - Provide ongoing support and advocacy for clients and their families - Collaborate with community resources to ensure access to necessary services - Monitor client progress and make adjustments to service plans as needed - Maintain accurate and up-to-date case records - Provide crisis intervention when necessary Qualifications: - Bachelor's degree in social work, psychology, or a related field (Master's degree preferred) - Experience working with individuals with developmental disabilities - Knowledge of behavioral health principles and interventions - Strong program management skills - Ability to supervise and provide guidance to direct care staff - Excellent communication and relationship management skills - Proficient in administrative tasks such as documentation, budgeting, and program development If you are passionate about making a difference in the lives of individuals with developmental disabilities, we encourage you to apply. Expected hours: 35 – 40 per week
