Reports To: Manager of Community Integration with dotted line reporting responsibility to the Manager of Transportation
Position Summary
Implements and monitors person-centered community integration services and habilitation outcomes for people with developmental disabilities. Uses a holistic approach focused on improving and maintaining independent living skills, as well as focusing on community membership, integration, advocacy, and pre-vocational training.
Essential Functions
Reasonable accommodations may be made to enable individuals with disabilities to perform these essential functions.
Implements and monitors person-centered plans in areas such as self-care (i.e. grooming, toileting, eating, dressing), communication, socialization, safety, motor skills, soft skills, sensory support, pre-vocational skills, daily living skills, community experiences, and other specialized areas based upon the outcomes listed in the Person Centered Plan (PCP).
Assists and supports those served in the attainment and implementation of personalized outcomes.
Observes behavior and participates in program activities. Uses on the job knowledge and data to make suggestions to the Manager of Community Integration Services and the Community Integration Coordinator.
Provides behavior support to persons served having difficulty to that they can maintain appropriate social interaction with other, and refrain from infringing on their rights.
Learns and utilizes all special and/or adaptive equipment with proficiency for those served.
Schedules, accompanies, and provides transportation into the community for outings as scheduled. This includes operating agency vehicles.
Secures the cooperation of individuals in assigned activities.
Completes documentation during each shift according to agency procedure.
Administers medication and medical treatment according to documented procedures. Follows Delegated nursing guidelines in the administration of all medicine and treatments.
Maintains the appearance and cleanliness of the site.
Interacts at all times with those served in a manner that denotes respect for personal integrity.
Represents Siffrin and those served in a positive and professional manner.
Maintains safe and sanitary conditions within the site at all times. This includes completing safety drills and checks per agency guidelines.
Completes additional duties as assigned by the Community Integration Coordinator, Manager of Community Integration Services, Director of Community Integration & Transportation, and/or Regional Director.
Follows Siffrin’s Code of Ethics in the fulfillment of duties.
Completes and maintains at least ten hours of ongoing training per year. This includes all required agency training: Delegated Nursing, CPR/First Aid, Bloodborne Pathogens, Mandt, Fire and Emergency Weather Training, Confidentiality, Unusual and Major Unusual Incidents, and Individual Rights.
Physical and Environmental Requirements
Must be able to lift at least 50 pounds; weight may vary depending on the location.
Must be able to push wheelchairs, use Hoyer lifts, bend, twist, and pick up objects with body and hands.
Must be able to operate a vehicle in all weather conditions.
Must pass a drug screen and alcohol test prior to hire and randomly on an annual basis.
Pass a DOT physical.
Qualifications
A High School Diploma or GED is required. A college degree in a related field of study is preferred, and/or comparable work experience will be considered.
One or more years working with people with developmental disabilities is preferred.
Must be able to provide physical assistance to persons served up to/including lifting, transferring, changing, etc.
Must have the ability to exercise constructive discipline in a structured, professional environment.
Must have the ability to communicate effectively in written and oral form, as well as use basic computer skills up to/including Word, Outlook, and applying internet based knowledge for searches and general navigation.
Must exhibit knowledge of the concepts of empowerment and behavior management. Must become familiar with the standards as mandated by the Commission on Accreditation of Rehabilitation Facilities (CARF) and the Ohio Department of Developmental Disabilities.
Must meet the agency policy with respect to a criminal background and motor vehicle record checks.
Schedule
Routinely works Monday through Friday 7:30am to 3:30pm; hours may vary. The schedule is flexible based upon the needs of the participants and Community Integration Services.
May be temporarily or permanently reassigned to other Siffrin locations as needed to meet the needs of the agency and those served.
Interaction
The Manager of Community Integration directly supervises the Transportation Professional.
In the absence of the Manager of Community Integration, the Transportation Professional will be supervised by the Manager of Transportation.
The Manager of Community Integration and the Manager of Transportation are both supervised by the Director of Transportation and Community Integration.
