Sierra Health and Wellness is Hiring a Psychiatric Nurse Practitioner: Part-Time Near Concord, CA

Our inpatient facility houses 22 clients who receive specialized and personal care from our staff. Our newly remodeled location offers amenities to both our clients and staff.

Positions available :

  • Part-Time- 0.6 FTE Hybrid ( Monday- Friday Partial Day, 25 hours a week)

Duties and Responsibilities:

The Psychiatric Nurse Practitioner, will function within any program operated by Sierra Health & Wellness in accordance with State licensure, Programs operated by SHW include residential substance use treatment facilities, day treatment programs, and intensive outpatient programs. The Nurse Practitioner will be primarily responsible for overseeing the medical management of clients with substance use disorder, mental health conditions, behavioral health conditions, and chronic medical conditions in accordance with standardized procedures and that of supervising physician staff. The Nurse Practitioner will assess clients, Initiate, or modify treatment within his/her scope of practice and seek consultation with supervising physician staff when clinically indicated. The Nurse Practitioner will participate in treatment team meetings with multidisciplinary staff members and participate in treatment planning for assigned clients. The Psych Nurse Practitioner will primarily provide services in person but may be delivered virtually when necessary. The Nurse Practitioner will participate in rotating weekly on-call services. On-call services will cover a staff on-site that are within nursing professionals' scope of practice to ensure appropriate care is delivered in a timely manner.

  • Assume principal responsibility for the medical, psychiatric, and addiction care needs of clients under direction of the supervising physician.
  • Perform History & Physicals, review and update outside provider H&Ps, and/or complete initial psychiatric evaluations for new admissions to residential treatment centers.
  • Perform admission assessments and contribute to development of client-specific treatment plans.
  • Assess clients and order medications as necessary to treat identified medical, psychiatric, and/or substance use disorders.
  • Order and interpret specific laboratory studies when clinically indicated.
  • Coordinate with multidisciplinary treatment team members at SHW.
  • Coordinate with clients’ outside providers, when clinically indicated.
  • Counsel and educate clients on medical, psychiatric, and substance use conditions.
  • Provide health education to staff (formally and informally) and clients related to medications, psychiatric conditions, substance use disorders, and medical conditions.
  • Document in medical records in sufficient detail to identify problems, goals, interventions, outcomes, and effectiveness of treatment.
  • Consult with supervising physician for conditions outside of psychiatric nurse practitioner scope and/or refer to specialized health resources for treatment.
  • Utilize screening tools to monitor effectiveness of care provided.
  • Adhere to infection control measures and maintain a safe environment of care for patients and staff.
  • Sign, date, and perform all charting, record keeping, and care plans in accordance with established policies and procedures.
  • Provide high-quality client-centered care.
  • Validate medication orders are transmitted to pharmacies electronically, or via fax/phone when electronic health record downtimes occur.
  • Ensure compliance with all medical/nursing and regulations for DHCS and Joint Commission and reordered from vendors as needed.

Requirements

  • Current Registered Nursing (RN) License required
  • Bachelor's Degree from an accredited University required
  • Master's Degree from an accredited nurse practitioner program
  • Active registration as a Nurse Practitioner by the California Board of Registered Nursing
  • Certification from a national accrediting body, such as the American Nurses Credentialing Center (ANCC) (e.g. Family Nurse Practitioner, Psychiatric-Mental Health Nurse Practitioner, or Psychiatric Mental Health Nurse Practitioner-PMHNP, etc.)
    • Alternatively, eligibility within 90-days of hire
  • Active Nurse Practitioner Furnishing license
    • Alternatively, eligibility within 90-days of hire
  • Active DEA number
    • Alternatively, eligibility within 90-days of hire
  • Active Basic Life Support (BLS) certification
  • Demonstrated experience and education in providing care for clients with mental health and substance use disorders.
  • Other duties as assigned.

  • Must have valid Driver's License
  • Adept counseling abilities
  • Ability to coordinate with professionals from diverse backgrounds

Hourly: $76.00- $114.00
