Join us at ShorePoint Health, where we expect nothing less.Job Requirements:* FL RN (Registered Nurse) license* Education: Associates Degree in Nursing* Experience: Minimum of three years of Acute Care RN experience within a hospital setting required.* Minimum 2 Years of RN Case Management within a Large Acute Care "Hospital".* 1 year of Utilization Review experience requiredAbout the Job:* Working closely with the collaborative care team, the case manager assesses plans, intervenes, and reassesses an individual's needs to implement a plan of care to meet the needs of that individual* The case manager assesses patient and family for clinical appropriateness for the setting and establishes plans for effective allocation of resources and management of the continuum of care.* Utilization review is conducted according to ShorePoint Health guidelines* Coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge* Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively.* Day Shift Available (8X5's varying days)INDBFH
