Sholom Community Alliance is Hiring a Registered Nurse Coordinator - Home Care Near Park, MN

 

SUMMARY

The Registered Nurse (RN) Coordinator, Home Care provides quality care for clients receiving Home Care services, provides direction and supervision to Home Health Aides and Homemakers, and in conjunction with the Home Care Director ensures compliance with state and federal regulations including policies and practices set forth by Sholom.

Shift

8 hours AM (schedule flexible)

PRIMARY RESPONSIBILITIES

  • The RN Coordinator complements the Director of Homecare Services in planning, developing and supervising the activities of licensed and unlicensed staff. Ensuring duties and responsibilities are accomplished for clients in a timely, professional, respectful and dignified manner. Perform supervisory visits under Minnesota Home Care rule guidelines. 
    • Maintain and ensure compliance for comprehensive documentation and reports per policy and regulation. 
    • Attend, and as needed, lead care conference meetings and participate in IDT (Interdisciplinary Team) meetings as needed. 
    • Share the responsibility in the training, monitoring, and evaluation of job performance of licensed and unlicensed staff. 
    • Organize, provide work direction and provide communication regarding expectations. 
    • Serve as clinical recourse to staff regarding care of residents. Provide education as needed. 
    • Act as advocate to maintain resident privacy and dignity. Administer medications and treatments as ordered. 
    • Monitor vital signs and complete resident assessments. Comprehensively assess resident change in condition and communicate to provider and responsible party as needed. 
  • Coordinate overall quality care for the clients, to include the following: 
    • Assess clients and report any changes in physical, social or emotional condition. Communicate in a timely manner with physicians and nurse practitioner’s changes of condition requiring medical interventions. 
    • Oversee ordering and set-up of client medications. Provide support assistance as needed 
    • Complete new admissions, hospital transfers and discharges 
    • Complete comprehensive resident assessment. 
  • Participate in the completion and review of client care plans. 
    • Over see the completion of Doctor and Nurse Practitioner orders to include ensuring accuracy and timely transcribing of the orders. 
    • Complete required evaluations, forms, events, charting and documentation, and ensure department compliance in accordance with Home Care regulations and Sholom policies and procedures. 
    • Identify and respond appropriately to emergency situations. Initiate emergency treatment, report, and address any safety hazards that exist in clients’ habits, activities of daily living, and the building environment. 
    • Update service plans and service contracts as needed. 
    • Perform admission and change of condition assessments.
  • Ensure proper billing practices and procedures. 
  • In the absence of the Director of Homecare Services, performs duties of the Director. 
    • Responsible for the performance management of the Registered Nurses, Licensed Practical Nurses, Home Health Aides and Office Coordinators.
    • Conduct all staff meetings.
  • The RN Coordinator shares responsibility backing up for the Nursing staff on a 24-hour per day, 7 days per week basis. 
  • Support various other Nursing responsibilities as needed.

JOB SPECIFICATIONS

Education, Experience and Credentials

  • Must be licensed as a Registered Nurse in the State of Minnesota and must maintain licensure. 
  • Current certification in CPR preferred; upon hire if not certified, must obtain no later than 60 days from hire.
  • Two or more years of experience working with the geriatric population or in a similar care environment preferred.
  • Six Months of supervisor experience in a similar care environment preferred.

Knowledge, Skills and Abilities

  • Flexible and productive in an environment with regular interruptions.
  • Good general nursing skills and proficiency in homecare nursing. Interest in and desire to work with older adults with understanding and compassion. Initiative and judgment in determining the needs of residents and maintaining patient care delivery in conformance with recognized standards.
  • Able to adhere to the Homecare Bill of Rights, HIPAA, and the Nurse Practice Act.
  • Able to keep current on changes in the industry, including clinical and regulatory changes.
  • Able to comply with established Infection Control and Safety Guidelines.
  • Able to work as an effective and proactive team-player; understands the importance of supporting the organization, residents/families/customers and other Sholom employees.
  • Able to display high levels of customer service, responding promptly and thoroughly to the inquiries and needs of individuals both internal and external to the organization in accordance with our PROUD standards.
  • Excellent interpersonal skills to interact effectively with a variety of people and personalities inside and outside the organization. Able to effectively handle interactions at all levels and to respond appropriately in sensitive situations.
  • Possess effective problem solving skills.
  • Able to understand and follow facility and department safety and emergency procedures.
