**Job Overview:** We are seeking dedicated individuals for Community Habilitation Positions to provide support and assistance to individuals with diverse needs in their homes and communities. **Duties:** - Implement individualized service plans to support clients in achieving personal goals - Assist with daily living activities and personal care as needed - Provide transportation for clients to appointments, outings, and community activities - Support clients in medication administration and adherence - Utilize behavior management techniques when necessary - Engage clients in activities promoting socialization and skill development **Skills:** - Experience in data collection and documentation - Ability to administer medications following prescribed guidelines - Valid driver's license with a clean driving record - Knowledge of direct support practices - Background in special education or related field is a plus - Proficient in caregiving and assisting with Activities of Daily Living (ADLs) - Excellent communication skills to interact effectively with clients, families, and team members - Experience in childcare or working with individuals with developmental disabilities - Proficiency in behavior management techniques This position offers the opportunity to make a meaningful impact on individuals' lives while gaining valuable experience in the human services field.
