Sheppard Pratt is Hiring a Medication Coordinator, Residential Transition – Sykesville, MD Near Sykesville, MD

The Segue Residential Transition Program assists with helping individuals transition out of state psychiatric hospitals. Segue clients are clinically stable and ready to leave the hospital. The services provided assist clients with learning how to manage their illnesses and develop independent living skills.

What to expect.

This is an opportunity to provide support for mental health treatment. You will oversee medication management for clients, train staff in medication monitoring procedures, assess for potential crisis, and implement crisis intervention actions as appropriate.

Additional responsibilities will include:

  • Providing transportation to and attending clients’ medical appointments, as necessary. You will ensure client understanding and proper follow-up of prescribed medications.
  • Developing medication plans for all clients receiving services.
  • Providing education and linkage to clients regarding medication and health services.
  • Counseling clients on the potential side effects of medication.

What we need from you.

  • A high school diploma or GED.
  • A driver’s license with 3-points or less and access to an insured vehicle.
  • Knowledge of medications, side effects, therapeutic effects, and the ability to effectively communicate this knowledge.
