Integrated Behavioral Health Therapist- LGSW/LCSW/LICSW

Shenandoah Community Health (SCH) is seeking an experienced, communicative, and compassionate (LGSW/LCSW/LICSW) therapist to join our team to support the physical, emotional, and mental well-being of our patients. In this role, you will have the opportunity to collaborate with medical providers to address the underlying mental health issues that impede good physical health. You will have the opportunity to learn from and teach primary care health staff as together you address the whole health of patients. You will teach and utilize cognitive behavioral therapeutic techniques to help patients address their symptoms, their immediate needs and discern whether or not they need more intensive behavioral health intervention. This role requires someone who enjoys working closely with people not just clients, but also other members of our incredible team.SCH is a community health center and Primary Care Medical Home that offers access to primary care, behavioral health, women's health, and oral healthcare in the Shenandoah Valley of West Virginia and Virginia. Since 1976, our dedicated providers and support staff have worked to ensure our patients receive the highest quality, most comprehensive healthcare possible. 

GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:

Responsible for providing initial diagnostic interviews after patient hand-off from Primary Care (PC) staff, collaborating on treatment plans with primary care providers, providing appropriate therapeutic interventions, and completing medical records and billing documents. The candidate must be able to demonstrate the knowledge and skill necessary to provide care based on physical, psycho/social, educational, safety, and related criteria, appropriate to the age of the patients served in his/her assigned area. The skills and knowledge needed to provide such may be gained through education, training and experience.

Flexible work schedule and Clinical supervision is provided.

SPECIFIC DUTIES AND RESPONSIBILITIES:

  • Effectively performs initial diagnostic interview and accurately completes the initial diagnostic forms within the established time frame.
  • Collaborates on and shares with patient an appropriate plan of treatment based upon the initial interview, PCP input, and psychiatric evaluation and completes the documentation required in the medical record.
  • Provides crisis intervention.
  • Provides recommendations for hospitalization, residential or outpatient treatment, possibly including a more appropriate placement with another SCH therapist within the co-located Behavioral Health Services (BHS) Department. 
  • Communicates and collaborates with patients and staff in an age appropriate, culturally sensitive, non-threatening, productive manner.
  • Makes referrals to appropriate staff or case management services to include Social Services, SUD treatment, Domestic Violence Programs, and further evaluation for psychosocial, spiritual and cultural needs.
  • Provides individual, and family therapy based on the inter-disciplinary treatment plan, within privileges and scope of practice at SCH Primary Care & BHS, and following minimum standards of practice for diagnosis or problem identified.
  • Substance abuse evaluation and transfer to the appropriate team member.
  • Effectively coordinates and communicates with other providers involved in the treatment of the patient; attends multi-disciplinary BHS treatment meetings weekly and Primary Care team meetings, huddles and other meetings deemed appropriate by Primary Care leadership.
  • Presents new treatments and effectively coordinates with other team members.
  • Additional duties as assigned. 

QUALIFICATIONS:

  • Ability to function professionally within a multidisciplinary team.
  • Ability to demonstrate courteous and cooperative behavior with patients, families, co-workers, subordinates, supervisors, and other department staff.
  • Ability to maintain patient confidentiality and privacy at all times according to policies and procedures; uses discretion when discussing patient matters.
  • Solid communication skills, including listening and the ability to demonstrate empathy, rapport building, and patience.
  • Ability to educate primary care providers and staff about mental illnesses, assessments, treatments, comorbidities, and interventions to improve patient adherence and care.
  • Willingness and ability to regularly implement process improvement strategies to enhance teamwork and clinical care.
  • Ability to work comfortably in fast-paced PC setting with flexibility, and willingness to quickly adapt work as may be needed in order to serve the best interest of the patient.

EXPERIENCE & EDUCATION:

·Must have a Master's degree in the field of social work and hold a current West Virginia license to practice as an LGSW, LCSW or LICSW.

·Must have at least two years of experience in behavioral health, inpatient and/or outpatient.

·Dual diagnosis experience is required 

·Must have an understanding of and acceptance of the benefits and role of integrated care.

·Must have basic computer skills and some experience with concurrent insurance reviews

·Familiarity with reimbursement issues is preferred.

·Must maintain licensure and comply with continuing education requirements stipulated by applicable credentialing boards.

·Experience using cognitive behavioral therapies.Using them to address lifestyle change and wellness issues would be a plus.

·Experience in integrated behavioral health is a plus.
