Position Summary

Promotes individual and population health by leading community health planning initiatives. Responsible for using a collaborative approach to improve public health outcomes in Shelby County. Developing and implementing a community health assessment, community health improvement plan, and planning cycles in collaboration with community stakeholders, elected officials, and community partners.

Pay Grade: 51

Salary Commensurate with Experience and Education


Minimum Qualifications

100%

1. Three (3) years of experience in community outreach, health education, or related field; and

2. Bachelor’s degree from an accredited college or university in public health, health promotion, public administration, health care administration, or closely related field; OR

3. An equivalent combination of education and experience may be accepted.

4. Experience in a health-related setting preferred.

5. Experience in project coordination and project management preferred.

6. PROOF OF EDUCATION, TRAINING AND/OR EXPERIENCE IS REQUIRED.


Duties and Responsibilities

1. Serves as project lead and garners active involvement of community members and stakeholders in the Community Health Assessment/Community Health Improvement Plan and development process.

2. Plans, implements, and maintains data collection and analysis systems to monitor health status and identify community health problems.

3. Analyzes County health data, including impactful measures of county health rankings, and looks for viable opportunities for improvement.

4. Manages focus groups and conducts key informant interviews and utilizes data to develop interventions and plans that address policy and environmental change.

5. Develops, maintains, and facilitates a sustainable health improvement action plan, including specific strategies, milestone activities, timelines, partner commitments, and a reporting system.

6. Coordinates public health planning projects related to the Shelby County Health Department strategic plan.

7. Prepares and delivers reports and presentations to diverse audiences.

8. Participates in various committees and other user groups involved in public health planning.

9. Leads special strategic and operational projects assigned by leadership, ensuring effective development, implementation, risk and benefit analysis, and completion.

10. Performs other related duties as required or directed.

1. Knowledge of community health assessments, planning, and evaluation in assigned program areas with expertise in program planning techniques.

2. Excellent oral and written communication skills.

3. Ability to gather, analyze, organize, interpret, and report data related to health program planning.

4. Ability to convene, facilitate, and educate diverse groups.

5. Ability to work on multiple projects at one time.

6. Ability to solve problems internally to motivate, be detail-oriented, and multi-task.

7. Ability to work independently and in a group to accomplish tasks with excellent organizational and interpersonal skills.

8. Knowledge of SPSS statistical software.

9. Proficient in Microsoft, Excel, PowerBI, SharePoint, Outlook, PowerPoint, and other County software applications.


Special Requirements

1. Work will require attendance at meetings and activities on weekends and/or after regular work hours.

2. Work will require travel to locations outside of the office.
