Shannondale Health Care Center is seeking a Social Worker.

The Social Worker is responsible for working in concert with other health care professionals to ensure that the needs of residents and their families are met in a timely, appropriate manner and in accordance with state and federal regulations.

Responsibilities:

· Working with individual patients and families toward adjustment to skilled/long term care.

· Serving as an advocate for patients and their families.

· Coordinating discharge planning functions.

· Participating in developing patient care plans with individuals from other disciplines.

· Completing patient interviews and MDS’s.

· Coordinating hospice referrals and complete audits for required documentation.

· Developing and maintaining contacts with appropriate community agencies.

The successful candidate will possess excellent communication skills and a compassionate demeanor.

Qualifications:

· A minimum of one year of experience working as a Social Worker in a health care facility and experience with discharge planning is required.

· Knowledge of the admission/discharge process in a skilled nursing facility and the ability to anticipate needs of geriatrics.

· Experience working with Medicare Part A, B, C & D, Medicaid, Managed Care and other private insurances.
