Job Details:
- This position supports individuals we serve in Shared Living/Adult Foster Care settings in Massachusetts.
- In this role, you will provide case management services for the individuals we serve in our Shared Living/Adult Foster Care Program with the support of your Program Director.
- Provide support and oversight to the Shared Living/Adult Foster Care provider (Mentor).
- Bring your creative ideas and your passion for success; we’ll train you on the rest.
- $42-$44k.

- Coordinate the development, review, and implementation of services, case management, behavior intervention plans, and treatments provided to individuals served.
- Oversee all services provided to individuals and their families, monitor the quality of care delivered, ensure compliance with contractual terms, and monitor regulatory program compliance.
- Assist in the development of quality services and engaging activities that meet the individual served needs.
- Build and maintain relationships with families and external case managers.
- Bachelor’s degree in human services
- Current driver's license, car registration, and auto insurance.
- A reliable, responsible attitude and a compassionate approach.
