Job Title: Licensed Practical Nurse (LPN) - SNF/LTC

Location: Santa Fe, NM

Rate: Local Rate= $1116 | Travel Rate= $1620 Weekly

Shift: 36hrs - 3x12hrs - 6a-6p Days

Job Description:

We are currently seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our skilled nursing facility/long-term care (SNF/LTC) team in Santa Fe, NM. As an LPN in our facility, you will play a pivotal role in providing high-quality care to our elderly residents in a comfortable and supportive environment. Your primary responsibility will be to ensure the overall well-being and safety of our residents through competent nursing care and strong interpersonal skills.

Responsibilities:

  • Provide direct nursing care to residents based on the established care plans and assessment of their needs.
  • Administer medications, treatments, and procedures prescribed by physicians, and ensure the accurate documentation of these interventions.
  • Monitor resident conditions, report any changes to the Registered Nurse (RN) or physician, and assist in developing individualized care plans.
  • Collaborate with the interdisciplinary team including physicians, therapists, social workers, and other healthcare professionals to ensure a comprehensive approach to resident care.
  • Maintain accurate and up-to-date resident records, including medical history, medication administration, vital signs, and progress notes.
  • Assist residents with activities of daily living (ADLs) such as bathing, dressing, feeding, transferring, and toileting.
  • Promote a safe and clean environment by adhering to infection control policies and procedures.
  • Provide emotional support and comfort to residents and their families, displaying empathy and compassion.
  • Participate in educational programs and staff meetings to enhance professional growth and maintain licensure requirements.
  • Adhere to all federal, state, and facility regulations and standards of nursing practice.

Requirements:

  • Valid LPN license in Santa Fe, NM with no disciplinary actions or restrictions.
  • CPR certification.
  • Previous experience in long-term care or skilled nursing facility preferred.
  • Ability to work with minimal supervision and prioritize tasks effectively.
  • Excellent communication and interpersonal skills, with the ability to establish rapport with residents, families, and team members.
  • Strong observational and critical thinking skills.
  • Working knowledge of electronic medical record (EMR) systems.
  • Ability to work rotating shifts, including nights, weekends, and holidays.
