We are looking for a Medical Social Worker (MSW) to join our dynamic team, serving Tuolumne and Calaveras Counties and surrounding communities, to provide care and support to terminally ill patients.

Qualifications

• Graduate of Master’s program in Social Work, (MSW)
• Minimum of one year experience in home health, or hospice preferred.
• Understanding of hospice philosophy and psychosocial issues surrounding death and dying.
• Strong Verbal and written communication skills.
• Strong Computer Skills
• Possesses and maintains current CPR Certification
• Valid California Driver’s License with reliable transportation and Auto Insurance in compliance with California requirements.
