Job Title: Working Site Manager

Program: DD Residential

Location: Rose Hill CLA

Schedule: 24/7 On Call

  • Works from the CLA and provides direct care as needed.
  • Site Manager provides a broad range of case management to individuals served, families, and provides supervision to the direct care staff working at their sites.
  • Supervision of staff: scheduling, training, disciplinary actions
  • Provide Social Services/ Case Management to Individuals Served
  • Ensures compliance with applicable requirements and policies,
  • Participate in weekend on-call
  • Monitors individuals served progress toward ISP goals
  • Assists individuals served in locating and utilizing community resources including legal, medical, financial, and other referral services.
  • Follows Serenity & DBHDD policies
  • Submits receipts for individual purchases timely & according to policy.
  • Participate in ID treatment team meetings,
  • Accountability and Management of daily documentation including weekly review of the EMR system.
  • Ensures Individuals’ records are up-to date and fluid.
  • Ensure CRA staff maintain annual training
  • Supervision of medications to ensure that medication is accounted for and available in the home every day.
  • Management of individuals served doctor’s appointments and follows up; must be present for any appointment where the client is scheduled to see a specialist.
  • Management of financial books and individual’s cash on hand, to include resolving any discrepancies immediately upon discovery.
  • Ensure that Medicaid renewals, Social Security reauthorizations and food stamps (if applicable) are submitted in a timely manner.
  • Assist staff in balanced and healthy meal planning
  • Review all other documentation at home sites, and official record on a weekly basis to ensure all forms and needed information is accounted (i.e. goals, ISP’s, HRST’s, annual forms, etc.)
  • Availability and flexibility to resolve any issues with staff and individuals served.
  • Coordinate and help staff and individuals served plan person centered weekly activities.
  • Must attend at all ISP meetings.

Minimum Qualifications

Highschool Diploma. Preferred Bachelor’s Degree in a Social Service or Human Service field and 2 years’ experience in working with developmental disabilities.

Good writing skills, computer literate, ability to coordinate and organize the delivery of services, uses observational skills to remain aware of client behavior and physical condition.

Previous supervisor experience desired/preferable
