Job Description
This position has the potential to achieve an additional certification as a fitness instructor. This candidate will work in the PEERS (people experiencing enriched recovery support). This offers Peer support groups 5 hours a day Monday through Friday. This program also
provides skills training and connects individuals with resources that will allow Individuals to live, learn, work, and socialize in their environments of choice. PEERS Provide service interventions focusing on wellness, identification of strengths, self-advocacy in connection with goals as identified by the individual’s assessment and treatment plan. Provides services to designated/assigned peer groups. Attends scheduled treatment team meetings and staff meetings. Completes and submits all clinical documentation/weekly progress notes. Ensures assigned caseload of individuals and has an active authorization before providing services. Maintains certification as a CPS (Certified Peer Specialist). Collaborates with psychiatrist, nurse practitioners, Program Directors all within SBHS. Must be dependable / flexible.
The employee is regularly required to sit; use hands to feel and/or handle objects, tools, controls; reach with arms and hands; hear, and speak. The employee is occasionally required to stand, walk, stoop, kneel and crouch. The employee must regularly lift or move up to 10 pounds, and occasionally lift or move 24 pounds or more. The employee may encounter various levels of emotional strain or tension.
Minimum Qualifications
Must have a High School Diploma or GED. Must have a valid driver’s license and a good driving record.
Preferred Qualifications
Bachelors degree and one year of experience as a case manager. Experience in documentation through electronic medical records.
