Role and Responsibilities

  • Determine placement requirements.
    • Conduct member intake and assessments.
    • Identify Housing or Health Services in the system and support for non-housing services that a member may require.
    • Identify a plan of action and work with the member to acknowledge next steps and requirements.
    • Once Member Intake is complete, update the member file.
  • Assist enhanced care manager and members with the exchange of resource and referral information.
  • Manage members progress and goals.
  • Communicate and follow up with an enhanced care manager (ECM).
  • Collaborate with a serene Community support manager to administer member changes and information updates (case Assignment).
  • Prepare all data and enter data into company platforms.
  • Maintain knowledge of company requirements and platforms to facilitate efficient work.
  • Prepares for weekly meetings with team members to keep them updated on current intake numbers.
  • Prepares for internal stand-up meetings to keep staff updated on member status or updates to policies/procedures.
  • Complies with work rules and policies.
  • Performs other duties as assigned.
  • Ability to work with multiple providers and nurture relationship(s) between Serene Health and the providers we work with in a professional manner.
  • Mediacy between Partnered health plans and their members.
  • Utilize provider portal(s) to verify member status and services, update member status and verify member information.
  • Ability to prioritize and work efficiently through-out the work day with a strong attention to detail.
  • prepare documentation and reporting on intake numbers.
  • make assigned number of intake calls and work with operations manager to reach team goals.

Pay range

$19—$24 USD
