Patient Coordinator

The patient coordinator will serve to provide general assistance, information, referrals, and scheduling for individuals and families seeking Behavioral Health Services. The Patient Coordinator represents to the community the quality of service delivery provided through Behavioral Health Services and thus is required to have interpersonal skills, patience, and a strong ability for problem-solving in crisis or conflict situations. This individual will implement a screening and triage process to assist families in determining the necessary services. This position requires excellent communication and organizational skills, the ability to interact with all levels of Serene Health's staff, and a wide range of external contacts. Education and training in a mental health field, as well as strong skills in data entry and basic computer skills, are required.

Responsibilities:

  • Implement a screening and triage process to assist potential clients in determining the necessary services.
  • Answering, returning, scheduling client calls.
  • Maintaining a working relationship with managerial, administrative, and clinical staff to assure efficiency in the day-to-day responsibilities.
  • The ability to add, update, edit scheduling services.
  • Ability to complete all data entry for pre-admission including identifying information, the reason for referral, relevant history, and payor type information.
  • Identify emergent or complex cases and seek clinical review, as needed, prior to finalizing the patient's appointment.
  • Maintains confidentiality of all clinical and administrative information consistent with agency policy.
  • We are a drug-free workplace and pre-employment drug screening and background check is required.

Qualifications:

  • Basic computer skills
  • Bilingual (preferred)
  • Communication skills

Pay range

$18—$22 USD
