Sentara is Hiring a Community Health Worker Near Norfolk, VA

Sentara Norfolk General Hospital is hiring a Community Health Worker.

This position is full-time day shift, 40 hours per week with rotating weekends.

Requirements:

-High School Diploma or equivalent

-Valid Driver's License and reliable vehicle

-Familiar with Social Determinants of Health

-Familiar with Social Services and Justice Systems

-Experience working with individuals diagnosed with Mental Illness

-Awareness of Trauma Informed Practices

-Strong interview/assessment skills

-Bilingual in English and Spanish is preferred

-Experience working in a community-based setting preferred

-Knowledge of medical terminology preferred

-Flexible with schedule

The Community Health Worker works in collaboration with the Case Manager and other healthcare team members to provide comprehensive and coordinated care. Engages in interventions to improve self-management skills and improve quality of life for patients with chronic and/or complex illness. Provides education and behavior modification strategies and provides social support to patients and families. Acts as a patient advocate and healthcare navigator in assisting with resource allocation and management.
