Great part-time opportunity for an LSW!

SeniorsPlus has an immediate opening for a Part Time (20 hours/week) Case Manager working in our Lewiston, Norway or Wilton offices as an integral member of the Community Services team to provide comprehensive case management to older adults residing in Androscoggin, Franklin and Oxford Counties.

The Case Manager will assess client needs, develop individual care plans, advocate, counsel and assist in identifying and accessing support services by conducting comprehensive assessments of clients’ needs, strengths, goals, and current circumstances to develop individualized care plans.

Qualifications

• Bachelors Degree in Social Services or related field
• Licensed Social Worker
• One to three years community experience in case management, professional social work, nursing, gerontology, health or other related human services preferred
• Microsoft Office and experience with database software required.

Job Type: Part-time

Pay: $20.00 - $23.00 per hour
