Job Title: Support Worker

Responsibilities:
- Provide support and assistance to individuals with disabilities or special needs
- Assist with daily activities, including personal care, meal preparation, and household tasks
- Promote independence and encourage individuals to participate in community activities
- Implement individualized care plans based on the needs and goals of each individual
- Monitor and document individuals' progress and report any changes or concerns to the appropriate supervisor
- Collaborate with a team of professionals to ensure the overall well-being of the individuals

Qualifications:
- CNA, HCA or NAR with letter
- Previous experience working with individuals with disabilities or special needs is preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Compassionate and patient nature
- Reliable transportation and a valid driver's license
