We are seeking a compassionate and dedicated Licensed Practical Nurse to join our team. As an LPN, you will play a vital role in providing quality patient care and assisting in the overall coordination of healthcare services. Responsibilities:

- Perform and complete all documentation for nursing assessments, observations, and planning evaluation pertaining to clients

- Evaluate clients on a regular basis and/or as necessary to ensure client needs are consistent with the care plan

- Be aware of home safety issues, strive to accomplish all responsibilities in a safe manner, and consistently reinforce good safety attitude and practices displayed by other staff members

- Educate and train caregivers on healthcare procedures and personal-care techniques

- Ensure compliance with infection control protocols and safety standards

Requirements:

- Valid LPN license in the state of South Carolina

- Proven experience in case management, hospice care, or similar settings

- Basic life support (BLS) certification

- Excellent communication and interpersonal skills

- Strong attention to detail and organizational abilities
