The Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center. Our mission is to empower independence and autonomy for participants and their support systems through exceptional interdisciplinary and integrated geriatric care. PACE is an alternative to nursing home care and through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides, we coordinate and provide preventive, primary, acute, and long-term care services, so older individuals can continue living in the community.
Primary Purpose
Assist with recreational and therapeutic activities for participants of Senior Care Partners P.A.C.E.

  • Assist in personal care needs of participants in the center or in the participant’s home.
Duties and Responsibilities
Prior to being eligible to perform this job, all PCA’s will have training provided by Senior Care Partners P.A.C.E. in all the duties and responsibilities outlined below. PCA’s will demonstrate competence in performing the necessary skills and only perform those skills outlined on the plan of care.
  • Assists in the implementation of therapeutic activity programs to include group and individual recreational and social activities.
  • Dementia specific programming
  • Assists with engaging participants with advanced dementia in appropriate activities.
  • Assists with overseeing the daily flow of activities in the memory care space.
  • Facilitates activities that may include small group programming, one-on-one programming, and sensory stimulation.
  • Provides input to promote effective activity programming according to a participant’s abilities and preferences.
  • Assists with community outings and special events for Senior Care Partners P.A.C.E. participants.
  • Assists in developing a monthly calendar of events for participants.
  • Maintains general order in the activities area, including daily clean up, routine supply inventory and ordering supplies for the recreation department.
  • Serves as a resource to staff.
  • Documents in the participant’s medical record as required by policy.
  • Must care, support, and collaborate with everyone involved in participant's care including the participant’s family using the patient-family centered care approach.
  • May assist with delivery of supplies to participants homes as assigned by the Area Day Center Department Leader or designee
  • Assist participants with ADLs in the center
  • May assist participants with IADL’s, ADLs, and/or other appropriately assigned tasks in the participants home under Emergency Staffing situations as assigned by the Area Day Center Department Leader or designee.
  • Responsible for completing all required education and file requirements on time.
  • Keeps leader informed of problems or issues.
  • Performs other duties as assigned.
Leader Responsibilities
This position has no leader responsibilities.
Qualifications
  • Must possess, as a minimum, a high school diploma, or equivalent.
  • Must be a Certified Nursing Assistant or complete PCA training that is provided by Senior Care Partners P.A.C.E.
  • Education or training in geriatrics, including dementia preferred.
  • Experience working with cognitively impaired adults preferred.
  • Well-developed ability to show discretion and maintain confidentiality.
  • Strong communication skills, both written and verbal.
  • Detail oriented with good problem-solving skills.
  • Basic computer skills desired.
  • Minimum of one-year experience working with the frail, elderly, or long-term care population.

  • Must possess current ServSafe certification or willing and able to obtain as offered by Senior Care Partners P.A.C.E.
  • Possess current CPR certification by AHA or The American Red Cross or willing and able to obtain as offered by Senior Care Partners P.A.C.E.
  • Ability to participate in the Participant Care Assistant Training through Senior Care Partners P.A.C.E.
  • Immunizations and vaccines must be current or medical documentation must be provided from a medical provider with reason (s) to request waiver. (Hepatitis B, TB, MMR, Varicella, TDaP, Flu, COVID-19)
  • Must provide exceptional customer service in support of all staff, external partners, and participants.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to plan, organize, develop, implement, and interpret Senior Care Partners P.A.C.E. goals, objectives, policies, and procedures.
  • Must have patience, tact, a cheerful disposition, and enthusiasm, as well as the willingness to handle difficult participants.
  • Must have the ability to get along harmoniously with others in a team environment.
  • Possess the ability to collaborate effectively with other IDT members.
Work Environment
  • Has a visible presence while working in the center and is subject to frequent interruptions.
  • Communicates with staff, participants, caregivers, and other organizations as necessary.
  • Works beyond normal working hours and on weekends and holidays as necessary.
  • Possess the ability to coordinate leisure interests for participants.
  • May be subject to exposure to TB, AIDS, and/or Hepatitis B virus.
  • Subject to callback during emergency conditions (e.g., severe weather, evacuation, disaster, etc.).
  • Must have reliable transportation and be able to travel between centers as needed; current valid Michigan driver’s license and current valid vehicle insurance required.

  • Must be able to relate to and work with the ill, disabled, elderly, emotionally upset participants within the Senior Care Partners P.A.C.E. Program and/or center.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must be able to push, pull, move, sit or stand for long periods of time.
  • Lift up to 20 pounds as needed.
  • May be necessary to assist in the evacuation of participants during emergency and/or disaster situations.
