- Coordinate schedules with transportation
- Work closely with clinics to schedule participant appointments
- Communicate effectively with participants and families, and other Senior Care Partners PACE disciplines
- May have other scheduling duties as assigned
- Work closely with Enrollment teams in a timely manner to ensure optimal participant transition to Senior Care Partners PACE program
- Must possess, as a minimum, a high school diploma, or equivalent.
- One year of experience in a scheduling role preferred.
- Must have a high level of proficiency using a computer system, with the ability to efficiently utilize EMR templates and Microsoft Office Suite.
- Has visible presence while working in the center and is subject to frequent interruptions.
- Communicates with staff, participants, caregivers, and other organizations, as necessary.
- Works beyond normal working hours and on weekends and holidays, as necessary.
