Required:
• Minimum two years of experience as Registered Dietitian or Registered Nurse, in diabetes or related field required.
• Certified Diabetes Care and Education Specialist (CDCES) or within 2 years of hiring.
• Bachelor of Science in Nutrition or Nursing
• Current certification in Basic Life Support at the healthcare provider level.
• Experience with insulin pumps and continuous glucose monitoring preferred
Licensure/Certification:
• Licensed Registered Dietitian, a Registered Dietitian by the Commission on Dietetic Registration (CDR) or,
• Registered Nurse current license to practice in SC.
Responsibilities:
Clinical:
o Provides direct patient care and/or consultation using in-depth clinical knowledge and skills in area of Diabetes and pre-diabetes.
o Professional knowledge and skill as a diabetes and health educator within depth knowledge and understanding of the nature of diabetes and related issues and patient care concerns.
o Ability to investigate and analyze information and draw medically appropriate conclusions.
o Serves as a consultant to other disciplines on diabetes.
o Receives and assesses referrals from referring providers.
o Consults with multidisciplinary team as appropriate to develop, implement and evaluate care for patients and families.
o Educates patients and/or families on self-management skills to promote optimal level of chronic disease management and wellness, with diverse populations and represent the organization in a positive manner.
o Documents patient care activities in patient electronic medical record.
o Technical and clinical knowledge of diabetes medications, CGM, and insulin pumps. Possess the ability to train and give guidance on the use of diabetes medications, CGM, and insulin pumps.

Education
• Participates in programs to meet continuing education requirements.
• Provide formal education to both internal and external groups pertinent to diabetes.
• Develop and present educational materials in support of specific educational and behavioral goals and modify such materials to as needed to support individual patients.
