Job Description:

As a Home-Based Family Services Case Manager, you will play a crucial role in providing comprehensive support and assistance to families in need. You will work closely with families to assess their needs, develop individualized service plans, and connect them with resources and supports to address challenges and promote stability.

Home-Based Family Services Program:

Our Home-Based Family Services Program is a cornerstone of our commitment to accessible and effective mental health care. Our program is designed to:

  • Promote Family Well-Being: We work collaboratively with families to address a variety of concerns, including communication difficulties, conflict resolution, parenting challenges, and behavioral issues. By providing therapy in the home setting, we aim to strengthen family relationships and enhance overall well-being.
  • Tailor Interventions to Individual Needs: Our providers utilize evidence-based practices and personalized treatment approaches to meet the specific needs of each family.
