Position: Hospice Navigator and Community Liason

Location: Rochester, MN

Status: Part-time

FTE: 0.6, with potential for growth

Hours: 24 hours per week

Shift: Days, some early mornings and early evenings

Position Summary: The Hospice Navigator and Community Liaison serves as a point of contact for patients, families, and the community at large. This position is responsible for working with those who wish to enroll in our home-based hospice services and/or in our Hospice House. This individual will assist patients and families to understand what hospice care is, the financial aspects of hospice care, and other financial resources available. Additionally, this position is responsible for cultivating and sustaining relationships in the community and with referral sources.

Qualifications Include:

• Associate or bachelor’s degree in marketing, Business Administration, or health care related field and/or five years or greater applicable work experience.

• Experience in health care marketing management preferably in hospice care

• Must be comfortable in end-of-life clinical settings and with ill patients and caregivers and demonstrate excellent interpersonal skills and effective listening skills.

• Must have strong written and verbal communication skills, strong organizational skills, accurate and attentive to details, proficient keyboarding skills, and belief in the hospice concept.

• Daily access to a reliable and insured vehicle. Valid driver’s license with acceptable driving record per policy
