Scituate Light Caregivers LLC is Hiring a LPN-Home Health Aide Supervisor Near Stoughton, MA

Introduction:

We are seeking a compassionate and skilled Licensed Practical Nurse to join our team and oversee our home care aides and ensure high-quality care to our patients. The Licensed Practical Nurse will assess patients' health needs, develop and implement care plans, and provide ongoing support and monitoring to ensure that patients receive the best possible care. The nurse will also oversee the home health aides providing care to the consumers. The successful candidate will have excellent communication skills and be able to work collaboratively with other healthcare professionals. **Bilingual staff encouraged to apply**

Responsibilities:

  • Assess patients' health needs and develop and implement care plans

  • Monitor patients' vital signs and report any changes to the appropriate staff member

  • Communicate with patients and families to provide education and support

  • Collaborate with other healthcare professionals to coordinate care and ensure continuity of treatment

  • Document patients' progress and outcomes using electronic medical records

  • Stay up-to-date with current research and best practices in nursing

  • Participate in professional development opportunities to maintain and improve skills

  • Other duties as assigned

Qualifications:

  • Current state license as an Licensed Practical Nurse

  • Minimum of 1 year of experience as an Licensed Practical Nurse

  • Excellent communication and interpersonal skills

  • Ability to work collaboratively with other healthcare professionals

  • Strong problem-solving and critical thinking skills
