A program created by the Maryland Department of Health which includes a coalition of healthcare and aging service advocates, leaders and community organizations. This program's mission is to serve frail older adults in the West Baltimore community to lead connected, meaningful lives.

The Registered Dietitian role offers a unique opportunity for a new elderly-focused program in West Baltimore which consists of a framework of supportive, innovative, and expert leadership team and company.
