Job Description: Hospice Social Worker
Position Overview:
My client is a brand new hospice company in the West Chicago area that is looking for a Licensed Social Worker (MSW) to be a part of their compassionate hospice team. The MSW will provide comprehensive psychosocial support to patients and families facing end-of-life issues. Strong background in social work, counseling, and palliative care required.
Responsibilities:
- Conduct psychosocial assessments and develop care plans.
- Provide counseling and supportive interventions.
- Assist with advance care planning and decision support.
- Coordinate community resources and referrals.
- Offer bereavement support and follow-up care.
Qualifications:
- Master's degree in Social Work (MSW) required.
- LCSW or LMSW licensure in Illinois.
- 2 years of relevant experience.
- Strong understanding of grief, loss, and family dynamics.
- Excellent communication and interpersonal skills.
- Commitment to hospice and palliative care principles.
