Overview

Duchenne Muscular Dystrophy (DMD) Registered Nurse Coordinator - Stony Brook Children's Service, UFPC

Location: Commack

Schedule: Full Time

Days/Hours:Monday – Thursday; 7:30AM -8PM (Variable shifts); Friday 7:30AM-5PM. 2 Saturdays per month. (Flexibility to do five 8-hour shifts or four 10-hour shifts)

Salary Range: $83,678.40 - $104,499.20 (pay will be determined based on experience, preferred qualifications, and education)

Responsibilities

SUMMARY: This position requires the Clinical Care Coordinator to facilitate care coordination services for patients in need of enhanced care coordination services. Patients will receive timely, high quality and efficient health care and support services through the development of a multi-disciplinary care plan, self-management goals and referrals to both internal and external resources.

Job Responsibilities:

  • Therapeutic infusions that are instituted in the Pediatric PSP and radiology suites for NM patients:
    • Scheduling infusion dates.
    • Notifying Pharmacy and Pharma reps for medication delivery and proper handling.
    • Proper thaw out of therapeutic agents and overseeing and documenting proper administration of such therapeutics in PSP and IR 1-2x/week.
  • Attend PPMD or DMD sponsored educational events (1-2 times per year) to expand their intellectual understanding of the disease entities and to network with national leaders in the field.
  • Initiate and support a quarterly support group/parental advisory panel for families.
  • Work with patients both in person and over the phone to remind and review their plan of care and progress towards their Provide patient follow up appointments and needed outpatient testing orders.
  • Meet (virtually) with the leadership team (week after) pre- and post-clinic for a summation of clinic needs and tasks needed to be accomplished for the week.
  • Create monthly clinical summaries based on patients seen during that month in the clinic.
  • Obtain prior authorizations and help with letters of medical necessities.
  • Handle sporadic New York state appeals.
  • Help maintain spread sheet of all active patients.
  • Triage Cerner messages for nurse or specialty providers and contact families with any of their queries
  • Manage referrals, when needed, to appropriate agencies required to assist the client in achieving the goals and objectives identified in their care plan.
  • Serve as the system navigator and primary point of contact for patients and families, providing direct access for inquiries and addressing concerns.
  • Help patients navigate potential issues related to the healthcare system, financial constraints, or social barriers by facilitating solutions such as arranging interpreters, transportation services, or prescription assistance. Act as the primary contact, advocate, and source of information for patients, care teams, family/caregivers, payers, and community resources.
  • Facilitate patient access to appropriate medical and specialty providers.
  • Educate patient and family/caregiver(s) about relevant community resources.
  • Promote and facilitate the collaborative management of primary care and specialty providers through effective communication, timely inquiries, follow-ups, and integration of information into care plans, particularly focusing on transitions-in-care and referrals. Additionally, identify and leverage cultural and community resources to enhance patient care and support.
  • Establish and maintain relationships with identified service providers.
  • Ensure timely documentation in Electronic Health Records.
  • Participate in Performance Improvement/Continuous Quality Improvement activities, as a

Qualifications

Required Qualifications:

  • Associate’s degree in Nursing. (New York State BSN in 10 Law - Must attain Bachelor’s in Nursing or higher within 10 years of initial licensure. Current RNs, as well as those currently enrolled or pending acceptance into a program preparing registered nurses effective date of this act (which is Dec. 19, 2017) are grandfathered in).
  • Current registration to practice as a Licensed Registered Nurse in New York State.
  • 3 years of clinical experience.
  • Highly organized with the ability to keep accurate notes and records.
  • Strong written and verbal communication skills.
  • Teamwork orientation.
  • Able to take and follow through with delegated tasks and accountability.
  • Resourcefulness in problem solving.

Preferred Qualifications:

  • Pediatrics experience.
  • Bachelor’s degree in Nursing.
  • Local knowledge about and connections to community health care and social welfare resources.
  • Experience with health IT systems and reports.

Physical Demands:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to communicate with patients, staff and medical providers. The employee must be able to exchange accurate information in these situations. This position is largely sedentary and requires the employee to remain stationary for a majority of the day. Any additional physical demands will be outlined and provided by management.
