Savannah Chatham County Public School System is Hiring a Community Navigator Project PREVENT Near Savannah, GA

This position is open until filled.

PRIMARY FUNCTION: The Poject PREVENT Community Navigator will focus on supporting children, youth, and their families at assign schools. Participating schools are Gadsden Elementary, Hodge Elementary, DeRenne Middle School, Myers Middle School, and Beach High School.

REPORTS TO: Director of Counseling Services 

SALARY SCHEDULE:PS-5

 WORK DAYS: 190 

REQUIREMENTS:

  1. Education Level: Bachelor’s degree from an accredited college; or university with two or more years’ experience providing specialized support or an equivalent of
    education and experience to successfully perform the essential job duties

Preferred: Master's degree from an accredited college or university in Social Work, Human Services or Education

  1. Experience, Skill, and Certification:
  • At least 5 years direct experience working with children, youth, and families
  • Experience networking and building partnerships with community organizations
  • Ability to organize, prioritize and manage multiple activities and tasks
  • Excellent verbal and written communications skills Ability to work independently

Preferred: Extensive knowledge of community resources and services targeted for children and youth and their families, especially those facing food insecurity, homelessness, mental health needs/concerns, and other barriers to education and overall health

ESSENTIAL DUTIES:

  1. Serve as the point of contact for students and families in need of assistance with navigating and accessing necessary community resources
  2. Develop, maintain, and nurture relationships with children and youth, their families, school staff and community stakeholders.
  3. Meet with and assess the needs of children, youth and their families
  4. Travel to various locations to assess needs, to include, emergency shelters, transitional housing, motels, cars, etc.
  5. Maintain detailed and accurate records of services provided
  6. Coordinate with community partners to assist with removing barriers to housing, legal, medical, financial, and mental health assistance for children, youth and their families
  7. Assist with disseminating information (posters, brochures, flyers) to schools and community partners
  8. Represent the department and school district to the community in a positive manner
  9. Attend and participate in scheduled staff development and district and department events, as needed
  10. Performs other grant approved duties as necessary for the effectiveness of the organization

TERMS OF EMPLOYMENT

Incumbents will be considered "at will." Appropriate pay will be determined based on Grade PS-5 and allowable experience as determined by Human Resources. This is a 190-workday position.

This position is funded through Project PREVENT grant and is contingent on availability of funds. 10/2022
