Position Title: Home Health Physical Therapist (PT)

Job Description:

A Physical Therapist administers skilled therapy care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and a plan of care under the direction and supervision of the Director of Clinical Services/Nursing Supervisor.

Qualifications:

• Graduate of an approved school of physical therapy and currently licensed in the state(s) in which practicing.
• Two (2) years of physical therapy experience, preferred.
• Experience with home health settings, including familiarity with Medicare regulations and OASIS documentation.
• Acceptance of the philosophy and goals of this Agency.
• Ability to exercise initiative and independent judgment.

Responsibilities:

• Provides therapy services in accordance with the plan of care.
• Makes the initial evaluation visit and regularly reevaluates the patient’s therapy needs.
• Initiates the plan of care and necessary revisions.
• Provides those services requiring substantial specialized therapy skills.
• Initiates appropriate preventive and rehabilitative therapy procedures.
• Prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.
• Coordinates services.
• Informs personnel of changes in the condition and needs of the patient.
• Counsels the patient and family/significant others in meeting therapy and related needs.
• Participates in and presents in-service programs.
• Understands and adheres to established Agency policies and procedures.
• Completes certification/recertification orders and discharge summaries.
• Determines the amount and type of therapy needed by each individual patient.
• Refers to other medical professionals (such as Occupational Therapists, Speech Language Pathologists, Medical Social Workers) as needed.
• Supervises and teaches other therapy personnel.
• Conducts patient care conferences on patients assigned to his/her care.
• Participates in peer review and performance improvement as assigned.
• Provides total patient care as needed.

Education:

• Bachelor’s or Master’s Degree in Physical Therapy (Preferred)

Experience:

• Physical Therapy: 1 year (Preferred)

License/Certification:

• BLS Certification (Required)
• Physical Therapy License (Required)

Work Location: On the road; Multiple locations

Job Types: Part-time, Contract

Pay: $50.00 - $100.00 per hour
