JOB DESCRIPTION SUMMARY The Care Associate performs various personal care services for the resident to meet the resident’s personal needs. The Care Associate is responsible for observing the residents, and documenting these observations and care performed. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES The Care Associate must exhibit the following competencies

SPECIFIC DUTIES AND RESPONSIBILITIES (not an allinclusive list)
1.ActivitiesFollows the required daily activities schedule and leads in one activity daily as designated by the LifeStyle Director and/or designee.2.Resident CarePerforming personal care needs outlined in service plan and attending to the resident’s and family requests promptly.Meeting safety needs of resident and using equipment safely and properly (transfer devices, etc.).Giving personal care including but not limited to baths (bed, tub, or shower), oral hygiene, hair shampoo, nail and skin care, assisting in dressing and undressing, and changing bed linens as assigned in service plan.Providing proper care and observation of residents skin to prevent breakdown and reporting when skin breakdown occurs immediately a licensed nurse and/or designee.Reporting on residents condition and significant changes to the licensed nurse and/or designee. Keeping residents living areas clean and orderly.Respectful of resident and family/caregiver environment and residents personal needs.Dining Services duties, i.e. taking orders, serving food, cleaning and maintaining dining area.
3.CommunicationMaintains regular communication with the Resident Service Coordinator and/or Resident ServiceSupervisor to review residents and help with managing service needs needs.Communicates needs for a change in service or if services are not showing on the service plan tothe Resident Service Coordinator.Promotes resident independence, respects privacy, and maintains residents’ rights to decideabout care and reports findings to supervisor.Works in cooperation with the family and outside providers to meet the physical/emotional needsof the resident.4.Additional DutiesCompletes documentation to ensure compliance with regulatory standards of timeliness,accuracy, and completeness via electronic record.Fulfills the obligation of requested and/or accepted resident assignments.Participates in all mandatory staff inservices, meeting, and/or education.Demonstrates familiarity with the policies of the Community and rules/regulations of state andfederal licensing agencies.
QUALIFICATIONS
High School Diploma or GED required.
C.N.A. Preferred
One (1) year of experience preferred but not required.
Must be computer proficient in typing and various programs, including background in EMR.
Ability to work with others as an effective team builder and team player.
Ability to take initiative, set priorities, multitask, and organize work.
Excellent communication skills, including public speaking, both verbal and written.
Subject to criminal background check and drug screening.
PHYSICAL/SENSORY REQUIREMENTS (with or without the aid of mechanical devices)
Standing, sitting, walking, lifting up to 50 lbs. on a frequent basis. Has well developed motor coordination and manual dexterity to perform basic skills. Must have good vision, hearing, speech and language abilities to perform and communicate Care Associate activities.
